For convenience and flexibility, attached are three formats of the Player Application: (1) a PDF version (2) a Microsoft Word DOCX version and (3) an ODT Open Office version. The PDF may be printed, completed by hand and mailled per the instructions at the top of the form. The DOC file or ODT file may be downloaded and comleted on your computer, then saved and attached to an email according to the instructions at the top of the form.
